Durable Equipment Specialist
Overview
Supervisor: Program Director
FLSA Status: Non-Exempt
Salary: $15 per hour- Part-time
Purpose: The Durable Equipment Specialist is responsible for refurbishing, delivering, and picking up medical equipment, and delivering food.
Essential Duties and Responsibilities
- Adheres to and implements all company policies and procedures
- Delivers, sets up and maintains equipment at the delivery address using company vehicle
- Maintains assigned company vehicle in a clean and safe working condition
- Properly cares for & uses electronic tablet that is assigned for job duties
- Provide clear instructions to consumers to ensure understanding of equipment
- Loads and unloads product in an orderly and secure manner in accordance with set policies and procedure
- Completes vehicles safety checks in accordance with set procedures
- Drives safely and defensively obeying all traffic law
- Provides safe environment for the patient at all time
- Picks up donated medical equipment, refurbishes equipment and delivers to consumers
- Ensures obtainment of proper signatures and delivery of (CAP’s) form
- Submits completed consumer packets to the program Director in a timely manner
- Maintains warehouse and inventory update
- Maintains delivery van and operates in accordance with local and state laws and regulations
- Maintains vehicle mileage log daily and submits monthly to Program Director
- Track’s gasoline and other van maintenance
- Cares for SCIL property within warehouse
- Sorts, bags and delivers food every week
- Performs other duties as assigned

Qualifications
- Background check
- Drug screen (when applicable for the position)
- Valid driver’s license in state of residence with a clean driving record (when applicable for the position)
Education and/or Experience
- High school diploma or GED equivalent
- 3 year’s delivery experience
- Strong customer service background
Skills, Knowledge and Abilities
- Effectively communicate in English; both oral and written. Spanish a plus.
- Helpful, knowledgeable and polite while maintaining a positive attitude
- Interpret a variety of instructions in a variety of communication mediums
- Knowledge of Durable Medical Equipment (DME)
- Maintain confidentiality and practice discretion and caution when handling sensitive information
- Mapping skills including ability to plan time-saving delivery routes

- Properly work an electronic tablet device that is used for services after training

Physical Demands
- Requires sitting, walking, standing, talking or listening

- Requires the ability to load, push, pull wheelchairs, power wheelchairs, scooters, hospital bed parts, walkers, commodes and other home medical equipment and supplies in and out of location, warehouse, company vehicle, consumer residence and/or delivery address

Disaster Response Case Manager
Overview

Purpose: Disaster Response Case Manager
Essential Duties and Responsibilities
- The consumer and Disaster Response Case Manager will work together to assess the consumer’s disaster caused needs and based on those needs, will develop a goal-oriented, to meet the recovery goal(s). Priority will be established at the time of the intake and will be based on basic needs for shelter, food, home modification/ramps, provision of Durable Medical Equipment/ Assistive Technology, provision of medicines (no cover by insurance) transition to avoid institutionalization, compromised health and well-being, transportation, and Personal Care Attendance services.
- Disaster Response Case Manager will advocate for the resources necessary to meet the needs and accomplishment of consumer’s goal.
- Disaster Response Case Manager will provide support and advocacy on behalf of the consumers with governmental and non-governmental agencies and organizations as needed.
- Disaster Response Case Manager will be networking with other organizations/agencies such as Federal Emergency Management Agency (FEMA), State Emergency Preparedness, multi-agency shelter transition, Florida Division of Emergency Management (FDEM), Red Cross and local official agencies to guide the consumer through the sequence of delivery without duplication of benefits or services.
- Disaster Response Case Manager will visit temporary housing/shelters, and any other institutional settings to verify the possible unmet needs and provide institutional transition to the community as needed.
- Disaster Response Case Manager will monitor shelters/temporary dwellings for health, wellness, and safety.
- Disaster Response Case Manager will reach out to the undeserved population/people with disabilities in the rural areas and provide the services needed.
- Promote awareness of the availability of Hurricane recovery resources.
- Support resource development activities at the local levels through outreach and public information efforts.
- Collaborate with local, national, state government and disaster relief organizations/agencies to provide resources to disasters survivors.
- Identify and engage community-based organizations that serve vulnerable populations.
Qualifications
- Bachelor’s degree. Two years of experience in an agency/organization that provides human services to the community understanding and embracing Independent Living Philosophy and disability culture.
- Bi-lingual (Spanish) preferred.

Independent Living Advocate
Overview
Department: Independent Living Services
Supervisor: Program Director
FLSA Status: Non-Exempt
Salary: Hourly $15 (part-time)
Purpose: The Independent Living Advocate will follow the independent living philosophy providing a variety of independent living services including the core services of Advocacy, skills training, peer mentoring, informational and referrals and transition to persons with disabilities, leading to opportunities for successful independence in the community. Additional services are provided through resources and trainings to obtain benefits, employment, education, medical services, and assistive technology.
Essential Duties and Responsibilities
- Complete an intake process, assessing their needs and developing a comprehensive Independent Living Plan (ILP) which establish goals and a time guideline assuring an efficient delivery of services and a reasonable time to achieve consumer’s goal.
- Provide a copy of the ILP to the consumer, as well as follow-up meetings to assess progress toward goal(s).
- Promote consumer choice and consumer control.
- Monitor progress of consumers towards developed goal, documenting with notes in their consumer files (CSR). Maintain consumers files in accordance with SCIL policies and procedures, data base and grants compliances.
- Input and maintain accurate real time consumer data in COMs database on regular bases as needed.
- Advocate for and assist consumers in obtaining benefits such as Social Security, Medicare, Food Stamps, and any other county, state, and federal benefits.
- Assist consumers in obtaining transportation services including fixed routes, Paratransit, etc.
- Assist consumers to obtain assistive technology.
- Provide direction to consumers in applying/obtaining personal care attendance.
- Provide direction to consumers to obtain employment or/and education services including Vocational Rehabilitation.
- Show awareness of, and participate in, systems change advocacy.
- Support grassroots advocacy efforts
- Assist with applications process as needed.
- Identify and participate in trainings/staff development activities/workshops to continuously improve knowledge and skills as it relates to independent living.
- Represent SCIL at outreach and events to educate the community about people with disabilities.
